Update on respiratory infections for the summer: COVID-19, influenza, mpox and more
Session moderated by Bob Wachter
In our final Grand Rounds of the academic year, we'll delve into the current epidemiology of COVID-19 and H5N1 influenza A in the United States, among other viruses. We’ll review the latest data and trends, this summer's "forecast," and how individuals and institutions can better prepare for prevention and mitigation. The session will begin with an epidemiology update with George Rutherford and then move to a Q&A with Alison Bond, Peter Chin-Hong, and Deborah Yokoe on H5N1, COVID, measles, and beyond.

Peter Chin-Hong, MD, is professor of medicine, Division of Infectious Diseases at UCSF Health and associate dean for regional campuses, UCSF School of Medicine.
George Rutherford, MD, is professor of epidemiology, preventive medicine, pediatrics and history and director, Center for Global Strategic Information and Public Health Practice, Institute for Global Health Sciences.

    ""Intracranial hemorrhage after use of tissue plasminogen activator for coronary thrombolysis." [Boston University School of Medicine] reports a significant number of cases (especially significant to the patients so affected) in which t-PA has induced brain hemorrhage, Kase [Boston University School of Medicine] "Ann Intern Med. 1990 Jan 1;112(1):17-21. The risk of blood-thinner-induced brain hemorrhage has not yet been taken seriously by the medical profession. My experience is of doctors mistaking for thrombotic stroke what was in fact antihypertensive-induced cerebral hypoperfusion in a patient with cerebral atherosclerosis, subsequently inducing cerebral hemorrhage by administering two (2) blood thinners. The same can happen with other cerebral space-occupying pathologiies.

    It was stated there was an "extremely low" mortality rate to the current strains of Covid. Can someone direct me to studies that have been done which show current Covid vaccines prevent transmission. I am confused. I thought it had been proven that now or at any time current Covid vaccines did not prevent transmission. Obviously my question is why are they recommending people be vaccinated with the current vaccines for Covid? Also why are they recommending that we mask again. Can they direct me to a single "legitimate" study that shows it "practically" works amongst everyday people in their hectic normal lives. Also can't you people develop a vaccine for Covid that does prevent transmission? I know why you can't and stupid as this sounds but if you're so clever then can't you prove me wrong. Also there is an immune detriment to repeated as you call it boosting. Are you not exhausting the immune system to do its other functions like cancer prevention. Can you not see that?

    I tried reporting that my partner and I had Covid last spring, and could find no way to do so. I left a msg w/my local health dept. in Oregon. We have it again, and I will try again, but I doubt anything's being recorded. I'm doing my part, but local agencies are not interested, and in my experience, have not been for some time.

    How can a new vaccine against H5N1 be tested where the prevalence in humans is so low? I assume Phase III human trials would be required. How long will it take to develop such a vaccine?

      @@MK-ih6wp Actually, don't take my words for truth. I don't know. I didn't get any covid vaccine myself but one thing I do know it's that vaccines do work. My dog was vaccinated against Parvo and when I had a stray dog with Parvo in my house for 15 days, my dog didn't get symptoms. I had that stray dog treated for 5 or 6 days then kept him in my house for recovery. There is no way my dog didn't get Parvo. The only think that helped is the vaccine he got.

    One of the reason the FDA selected the JN.1 subtype to be the target for the vaccine to be deployed in the Autumn/Fall, was because NOVAVAX could not be switched to the KN.2/KN.3 subtype in the required timescale. This suggests that the FDA should have decided which subtype to use at least a month ago. The US tends to vaccinate against Flu and COVID-19 from the end of October. In the UK we start to vaccinate at the beginning of September. In recent years this has meant that orders were delivered too late and we have had to use earlier versions of the vaccines. Autumn 2022 planned bivalent BA.4/BA.5 + original received bivalent BA.1/BA.2 Autumn 2023 planned monovalent XBB.1.1 (prefilled single dose AD syringe) received bivalent BA.4/BA.5 Spring 2023 planned bivalent BA.4/BA.5 + original received as planned Spring 2024 planned monovalent XBB.1.1 received as planned Hence those 65 to 74 did not receive the intended protection.

      Dunno if you heard yet, but the FDA just updated their recommendation, and are now suggesting vaccines go with KP.2 (though they acknowledge Novavax will still be JN.1). I get everything else you're saying, of course. This game of whack-a-mole isn't sustainable -- the virus is always going to outpace us.

    I started with vax in april 2021, then boost in 1 mo, again vax and boost, so 4 pokes, then one last in nov 2022, all moderna, it was 50mg, instead of 100mg. Then, nov 23, 2022, just 2 weeks after the 5th poke, i got covid. I got on azithromycin, paxlovid, and steroids. 8 months later long covid presented its ugly head. My antiphospholipids-beta 2 glyco protiens are sky high, i aquired mixed connective tissue disease, several auto antibodies, vasculitis, sever tinniitis, have gone through hundreds of blood tests,and trying to get an infectious disease doc who specializing in kong covid. I have taken 10 day taper predisone, got herpes, took 10 day aclyclovir, but nothing else yet today is may 26th 2024, sufferhng!!!!!!!

    Great video, valuable information, my only criticism is that it was MUCH too short and lacked the details we all want as patients and medical professionals. Any chance the next one can be 2 or 3 hours in length and much more in depth? I would also suggest that each researcher go very deep into details about their areas of expertise. For example on Dr. Peluso's blood work panels used for diagnosis more info on the numbers for proper Vitamin D3 blood levels, B12 levels and Thyroid panel would be useful. Is there any benefit in using high dose calcitriol or calcidiol in treatment for the severely ill in hospital over time? Maybe you could go through an anonymous patient workup on initial findings and end goal blood levels. We KNOW that low Vitamin D3 blood levels cause a severe deficit in the immune system, causing autoimmune states, and that restoring higher, proper Vitamin D3 blood levels regulate the immune system to a proper state of function. It seems to me that Long Covid is an autoimmune disease and ALL autoimmune disease are caused largely, not entirely, by malnutrition. Low Vitamin D3, low Vitamin C, low B12, low Thiamine - B1, low Omega-3, low steroid levels, low minerals like magnesium, potassium, selenium, folate, low iron, low iodine, etc...causing an entrance scenario ready for infection and inflammation. I also feel that current recommendations for Vitamin D3 blood levels are still MUCH too low. (100ng/ml sounds about right for a base level) Also how do you feel about Dr. Paul Marik's MATH+ protocol for Covid and Sepsis which includes steroids, ascorbic acid, thiamine and heparin amongst other essential vitamins and minerals. It seems even more appropriate now with the discovery of low steroid levels in long Covid patients. Finally is there any value to be gained in treating severe long Covid patients with IGIV and PLEX (immunoglobulin and plasmapheresis) as front line GBS (Guillen-Barre Syndrome) patients are initially treated to save their lives?     This is great info. I’m so frustrated that none of the (many) providers I see ever acknowledge the impact of long COVID on my health. I was hospitalized with COVID in October 2020 and nearly died and I was barely keeping my head above water after returning to work that December, and then a year and a half later I got diagnosed with ovarian cancer. I really think the combo of LC + cancer treatment was unbelievably debilitating, to say nothing of the (diagnosed) PTSD I have after those experiences. 😢

    My only concern re this fine discussion, is no mention re the dietary contribution for improving the body’s systems to deal with the impact of medications on the efficiency of system to recover…or is there a way to lose weight when so many meds seem to be making digestion, physical movement more challenging

    I am now dealing with Brain Fog, gait after two episodes of COVID in the fall of 2023. I did get vaccinated and one booster. I was hospitalized in Sept., not tested in the hospital but when discharged I tested myself and was positive for COVID. I don't remember my behavior when in the hospital, but my husband was concerned as I was non responsive and when in the the hospital the nurse caring for me increased my IV flow and I began to recover and became responsive. this has gone on for a long time and my Dr. wanted me to go to a neurologist,I followed up doing this ,but the Dr. is totally disregarding my history with COVID. So he is emphasizing Dementia. I am 78 and there may be some truth to this although I do not see this. I am independent totally in my home and outside the home. I do have the usual forgetfulness, but indicates a pathway to dementia in the short term. I do not want to go with medication, but would like to go with supplements and diet. I am very discouraged not with what I read and study through but with my Neurologist one track thinking . Thank you for hearing and may God lead all who are working for a solution to an answer.
